Parent Acknowledgement:
 

1. ​I have received a final NZBA activity statement with balance as at 30/3/2020.

2. I am aware as per the NZBA Financial Policy that NZBA have retained $350 to cover the non-refundable deposit for the flights.

3. I am aware that any outstanding NZBA payments have also been deducted from any refund owing.

4. I have provided the name and bank account no which the refund will be deposited into. 

5. I confirm that I am the legal guardian/caregiver of Student Athlete named above.

6. I am aware that this is a one off payment due to the unforeseen circumstances of
   COVID-19.
